Terms and Conditions for Dolce Vita Chiropractic

Welcome to Dolce Vita Chiropractic. These Terms and Conditions (“Terms”) govern your use of our services, website (if applicable), and your interactions with our clinic. By accessing or using our services, you agree to be bound by these Terms. If you do not agree with any part of these Terms, please do not use our services.


 

1. Acceptance of Terms

 

By becoming a patient, scheduling an appointment, or using any services provided by Dolce Vita Chiropractic, you acknowledge that you have read, understood, and agree to these Terms and Conditions. These Terms constitute a legally binding agreement between you and Dolce Vita Chiropractic.


 

2. Our Services

 

Dolce Vita Chiropractic provides chiropractic care, including but not limited to examinations, diagnoses, adjustments, therapies, and related wellness services. Our services are provided by licensed chiropractors and qualified staff in accordance with Texas law and professional standards.


 

3. Patient Responsibilities

 

As a patient of Dolce Vita Chiropractic, you agree to:

  • Provide Accurate Information: Furnish complete and accurate health information, including your medical history, symptoms, and insurance details, to facilitate appropriate care.
  • Adhere to Treatment Plans: Follow the treatment plan and recommendations provided by your chiropractor.
  • Keep Appointments: Attend scheduled appointments or provide timely notice for cancellations or rescheduling.
  • Financial Responsibility: Be responsible for payment for services rendered, including any co-pays, deductibles, or non-covered services, as outlined in our financial policy (which may be a separate document).
  • Respectful Conduct: Interact with our staff and other patients in a respectful and courteous manner.
  • Compliance with Policies: Abide by all clinic policies and procedures.

 

4. Appointments and Cancellations

 

  • Scheduling: Appointments can be scheduled by phone, in person, or via our online portal (if applicable).
  • Confirmation: We may send appointment reminders via phone, text, or email.
  • Cancellation Policy: We require 24 hours notice for any appointment cancellations or rescheduling. Failure to provide sufficient notice may result in a cancellation fee of $15. This policy helps us manage our schedule and serve all patients effectively.
  • Late Arrivals: Arriving late for an appointment may result in a shortened session to avoid delaying subsequent patients, or the need to reschedule your appointment to a later time.

 

5. Financial Policies

 

  • Fees: Our fees for services will be communicated to you prior to or at the time of service.
  • Insurance: We currently do not accept insurance plans. It is your responsibility to understand your insurance benefits, including co-pays, deductibles, and coverage limitations.  As a courtesy, we can provide you with a “Super Bill” that you can submit to your insurance claims department.  Ultimately you are responsible for payment of all services.
  • Payment: Payment for services is due at the time of service unless other arrangements have been made in advance. We accept the following payment methods: cash, credit cards, debit cards, health savings account cards].
  • Unpaid Balances: Unpaid balances may be subject to collection efforts, and you may be responsible for any associated collection costs or legal fees.

 

6. Privacy Policy

 

For your Documentation:  Your privacy is important to us. Our collection, use, and protection of your personal and health information are governed by our Privacy Policy, which is incorporated into these Terms by reference. Please review our Privacy Policy  for details on how your information is handled in accordance with HIPAA and the Texas Medical Records Privacy Act.

For your Treatment:  There are no individual rooms for privacy of treatment, we have an open floor therapy room where all the physical exercise, adjustments, and orthopedic exams are performed.  Part of the treatment may be captured by our security cameras.


 

7. Disclaimers

 

  • No Guarantees of Outcome: While we strive to provide the highest quality chiropractic care, we cannot guarantee specific outcomes or results from treatment. Your response to care may vary.
  • Not a Substitute for Medical Advice: The information provided by Dolce Vita Chiropractic, including on our website, is for general informational purposes only and does not constitute medical advice, diagnosis, or treatment. Always consult with a qualified healthcare professional for any health concerns.

Please note the following regarding our SMS communications:

  • No Protected Health Information (PHI) in Standard SMS: We will never include detailed Protected Health Information (PHI) within standard SMS text messages. We are committed to protecting your privacy in accordance with HIPAA and the Texas Medical Records Privacy Act. If sensitive health information needs to be communicated via text, it will only be done through a secure, encrypted messaging platform, and only if you have explicitly consented to the use of such a platform and its associated terms.
  • Consent to Receive Messages: By providing your mobile number, you give us express consent to send you text messages as described above. You understand that message and data rates may apply from your mobile carrier.
  • Opt-Out: You may opt-out of receiving SMS messages at any time by replying STOP to any message you receive from us. Please allow up to [e.g., 5 business days] for your opt-out request to be processed.

  • No Data Disclosures: Your mobile phone number and any information related to your SMS communications with Dolce Vita Chiropractic will be used solely for the purposes outlined in this section and our Privacy Policy. We will not sell, rent, or otherwise disclose your mobile phone number or text message data to any third parties for marketing or other unrelated purposes. We utilize a HIPAA-compliant messaging service provider that acts as our Business Associate and is contractually obligated to protect your data in accordance with federal and state privacy laws.

Important Consideration for PHI and SMS:

For any communications that involve Protected Health Information (PHI), we will primarily use secure methods such as phone calls, in-person discussions, secure patient portals, or secure, encrypted messaging applications that are specifically designed to be HIPAA compliant and for which you have provided explicit consent. If you initiate communication with us via standard SMS that includes PHI, you acknowledge the inherent risks associated with such unencrypted communication and agree that Dolce Vita Chiropractic is not responsible for the security of PHI sent by you through non-secure means.
